Fillable pdf forms created
Budget: $30 – $250 USD
I am looking for a freelancer to create fillable PDF forms for collecting customer information. Forms will be used internally only by staff during live calls. The forms will require more than 20 fields. Although I don't have a specific design in mind, I have some ideas that I would like to discuss with the freelancer. Attached are examples of what we currently use but I need to add more fields and streamline filling out as best as possible. Looking to make one for auto only, one for home only, one for both.
Skills and experience required for this project include:
