Fillable pdf form

I am looking to have a fillable pdf form created for Rapids Field Neuro Checksheet. The form will be used to collect customer information and its purpose is to create a survey. I have a specific design in mind and would like the form to comprise of 1-3 pages. Whoever is hired to complete this task should be detailed oriented, and should understand the required data that needs to be collected in an effective and efficient manner. The successful candidate should also be familiar with the latest software design techniques and have knowledge of relevant industry standards.
