Fillable PDF needed

I have a 7 page document that I need converted to a fillable PDF. Each page has 20-25 response fields that need a Yes or No answer along with a space to write a sentence or two. The form is already done, I just need to be able to fill it out on my computer rather than print and fill it out by hand. The form also needs to be able to accept initials and dates on the bottom of each page and allow for an e signature. I have attached the form for review,
