Fill out PDF file based on CSV data

My project takes a CSV file (in Unicode format) and needs to fill out a PDF file (as attached). There will be 2 pages which are the same. (i.e. we need to fill both pages with the same information and the fields are in the same positions in both pages).
Many of the fields in the attached PDF are not used. There are about 17 items we need to fill out in total (per page). Also to add 2 images (per page). Basically we just look at the CSV and if an element exists in the CSV we place it at the predesignated position in the PDF so the program has a list of field names or x and y co-ordinates to map the CSV text items to. The text size needs to balance to fit the fields and be readable.
If you can write this within Excel VBA this would be nice but I'm open to other language such as Python too.
