Editable PDF Assessment Report

I am looking for a freelancer to create four editable PDF assessment reports for player progress progress tracking. The PDF should include multi choice selections that can be filled in by the coach as well as fillable text fields where the players details can be entered and the coach can add any relevant comments. Digital signatures are also needed. We would also like for the design of the form to be improved to be something similar to the 'Draft RFC' attachment (with check boxes for scores and a better design). We will supply the imagery for this upon awarding the project and each of the four reports would need to only be one page long and not two, as shown with the 'Draft RFC' one. Ideal skills and experience for this job include proficiency in Adobe Acrobat and graphic design.
