Design Hotel Ledger

I need a hotel ledger/bill designed by somebody who would know how this would look. It just needs to be one sheet of paper, needs to show an opening balance, three months with the total of each month, a payment made, and the ending balance due. So somebody who is good at math and PDF editing. I have somewhat of a template to work from.
