Custom PDF Invoice Form Creation

I need a professional to create a PDF invoice form that I can give out to our workers so I can get consistent invoices that has all the information I require. I have had lots of people unable to use excel so I want a pdf form which anyone should be able to access and fill out.

Essential requirements:
- Client details - name, address, mobile, UTR no, NI no
- Billing details
- Bank details
- Total amount
- Tax deduction
- Prices
- Day rate
- Due date
- A drop down of our sites people have worked on
- Description of works
- Needs to fit on an A4 sheet
