Custom Fillable PDFs for Training Course

I'm on the hunt for an experienced freelancer who can come up with two fillable PDF forms for a machinery operation training course. The forms will be used for an assessment questionnaire aimed at gauging the understanding and knowledge of the trainees.
•The first PDF form needs to have a range of questions that the trainees can answer through tick boxes labeled 'yes', 'no', and 'N/A'. Alongside, the form must feature a button to verify that the responses are correct.
•The second form should only have a button to tick all the boxes.
The ideal candidate for this project must have experience in creating PDF forms, preferably fillable ones. Understanding of machinery operation or industrial training could be an advantage but is not required. If you have related examples of your previous work, I encourage you to share them.
