Create a pdf form, with FIllable form fields
Budget: £20 – £250 GBP
I have a master form, which I wish to edit/replicate. (SES LTD Subcontractor Registration)
We should retain colours and fonts.
I wish to have this form in "Fillable PDF form" style. I have attached an example (64-8).
The completed form should be able to easily be filled on a computer.
I have attached a word document which contains all the relevant text for the new form (Subcontractor Agreement template)
I have also attached a brief mockup which you may be able to interpret. (Mockup)
The form is essentially broken down into nine sections.
The straight lines on my mockup are the formatted text and the squiggly! Lines are the text boxes.
We will then look to add some promo/branding in the free space remaining.
