Create fillable forms out of these word files

I wish to create fillable forms out of a few word documents. The forms need to be locked ( only the fillable parts can be altered ). The word file linked to this offer is an exemple, the highlighted parts are the parts that need to be fillable. The checkboxes already in place also need to be modified into a fillable form.

I also need those files to not save over each other every time that someone fills them. In other words, the files need to be saved as a new copy every time, I don't want to corrupt the original version.
