Create a fillable PDF document

I am looking for a freelancer to create a fillable PDF document for me. The purpose of the document is to collect critical Tenant information as part of a rental application with our Company.

Please see attachments for additional changes that will be required prior to creation of the form.

Requirements:
-The documents should be viewable digitally for online use, and include text fields for users to input their information.
-The document should be able to be printed for offline use.
-The document must resemble the current application form (attached), and remain simple, and professional looking.

Ideal skills and experience for this project include:
- Proficiency in PDF design and creation
- Knowledge of form creation and functionality
- Attention to detail to ensure all fields are included accurately
- Ability to work efficiently and meet deadlines
