Create an editable PDF of a document - I only have photos

I have a driving instructor form that I need recreated and then exported as a editable pdf. The document is old (about 15 years old). I need you to create the document exactly.
The document labelled 'Checklist (1)' is the original form but it is missing the bottom section of the form.
The second document has the missing section from this form but is very low quality and is filled out.
I have attached the logo from the top left corner as well.
I need the form in the highest quality possible
