For example, Form f8938a-2020, Page 1, Field 1 (Name) should be copied to Form f8938a-2021, Page 1, Field 1 (Name). Another example, Form f8938a-2020, Page 3, Field 1 (Type of Account) should be copied to Form f8938a-2021, Page 2, Field 20 (Type of Account).
The PDFs must be filled using Adobe Acrobat Reader. Please confirm that you have Adobe Acrobat Reader. Thank you.
