Convert Word Form To Pdf Fillable Form

I have an already functioning Word-form (6 pages), which I would like to be converted into a PDF-form with tab-functions. The text is in Dutch, but not necessary to understand that.
Have several such forms of different number of pages so if it goes well I have more jobs.
