Convert Simple PDF into Fillable PDF, Microsoft word or Excell and Google doc or sheet.

I have this simple Employee review form that I would like to have made to be easily filled out online. I have attached the file here. It is a very simple one-page PDF for an employee review. Please convert this to something I can open using the Microsoft suite and also a format that I can open using the Google suite, as well as a fillable PDF.
