Convert 12 Page Form Images to PDF/Word
Budget: $10 – $30 USD
I have images of a 12 page form that I require to be retyped, and copies sent in PDF, and Word, The details are as follows:
1- Page cover sheet as follows:
- Logo applied to the page
- Wording applied to page: "Responsible Care and Standards, Procedures - Contractor Environment Program Information"
11- Pages Worksheet Pages:
- Each page has to have page # of 11 pre filled at the top right as is in the form.
- All check boxes need to be blank.
- All handwriting on the pages needs to be omited.
- Small Logo applied to the top right of each page.
The completed file is required within 24hrs.
