Convert PDF form to Word

I have a PDF form. When I try to edit it in MS Word, some of the tables turn to images.
I need you to convert it to a proper editable Word document with tables. If I need to add more rows, I should be able to just add more rows, and everything below the main table should adjust down.
The PDF and guidelines are attached. The first one to do it will be awarded the project.
