Convert Hard Copy Form to eForm

I need some existing PDF forms converted into an eForm. The eForm needs to have the same design as the PDF form. Additionally, the eForm should be delivered as a standalone PDF. It needs a signing and date section which is electronic along with boxes that can be selected - refer to the tick boxes.
