Convert Documents to Editable PDF Forms

I have some forms I need updated and converted to editable PDF Forms for a website. The final documents must be able to be opened and edited in Adobe Acrobat so I can edit the form content going forward.
The Health History form must have fillable forms and functional radio buttons for the yes and no questions.
