CONVERT DOC TO A FILLABLE PDF

I need a document converted into a fillable PDF. The form has already been created and needs a few edits and conversion.. he PDF should contain text fields and checkboxes, but no special formatting or branding is required. The fillable PDF does not need to contain any sections that require calculations or automatic data population, just basic fields.
