Application Form Design and Creation

We have created a first draft/template of an application form (in word and PDF) of which we need the design to be tweaked and cleaned up a bit with correct formatting etc, including tick boxes - produced in both word and PDF, which editable fields for saving, so that clients can pre-fill their information and sign.
