Adobe PDF form - email

I have a pdf form with several text boxes and dropdowns that employees use to fill in data. What I need is for one of the fields to ask a question - if the answer is no just save that answer as no. If the answer is yes then I need to receive an email telling me that this form had a yes answer and then save the answer on the form as yes. Example form is attached.
