Adobe Acrobat or Microsoft Fillable Form Creation

What I would like to do is create a service ticket that users (picked from a drop-down list) can fill in data (customer name, telephone number and reported problem) Dropdown List (building number and room or door number)
Save the data to the same file, Later recall ticket fill in work performed and then have customer sign it, save it back to the same file, be able to email it, print it, and keep the ticket forever. Have a way to find it later and update it if necessary. I would also like to have each new ticket auto number (ticket 001, ticket 002) Auto date (ticket created, ticket updated, ticket signed) Have a drop down of users (individuals who create tickets), Buildings, rooms doors plus any other miscellaneous data needed in a text box. A fillable area for work performed, Also, a drop-down list for parts each part will need to have a price attached (Item: door switch Price: $10.00) used, fillable area for parts not on the list. All data must be kept local on my shared drive. I also have Microsoft office 365 SharePoint for storage. a management area for adding users, items to drop down lists, modify entire form (rename fields add fields), if necessary, Logo placement area and for backup/recovery.
