Academic Membership form formatted and converted to a "fillable pdf" form
Budget: $30 – $250 USD
I have attached 2 files:
1. A 2 page application form in word format which is a "draft"
2. A 2 page similar application form which can be used as an example of what I would like the general format of the final result to look like.
Page 1of the Word Document is all text and does not require any formatting.
Page 2 needs formatting and needs to be "fillable" from top to bottom.
There are no "required" fields and there is no filtering required (all fields are text).
As deliverables, I would like
1. A final useable pdf document that is a "fillable pdf" form that is of similar format to the attaced fillable pdf.
