PCB design to be made from a circuit diagram

I am looking for a PCB design contractor to make a PCB design and create a PCB based off of an existing circuit diagram. The size of the required PCB is small (less than 10cm x 10cm). Additionally, I need the PCB to be tested after it is constructed. The type of PCB required is double-sided.
