PCB design for existing Arduino code

I have some Arduino code to open lockers electronically that uses a mega 2560.
I need a custom PCB designed for this board that will have 13 relays on it (to open 12 lockers and the last one turns off an input) and a connector which I can then plug the other outputs into. It has 3 pushbutton inputs and some outputs for the relays and LEDs. The relays only need to power 1.5a/12v DC each.
Need all of the required files to pass this onto a PCB manufacturing/assembly business.
