3G/LTE communication

I need a compact, low power device that registers a high output (12V) when a phone call is received. Power supply: 12V. The phone call must be received correctly in Spain (see 3G and LTE bands) with physical sim card. I've tried with SIM 800L device without success. Required: list of materials, operating diagram and devices wiring. PCB layout is not essential
