Custom battery pack

Hello i am working on custom battery pack . I have designed the pcb for the pcb using the texas ic BQ76920. I am looking someone to check the circuit diagram/schematic and the pcb parts and find any mistakes cause currently is not working properly. Specifically i have wrong current readings. Also in my last test i damaged the ic.
