Vehicle title transfer of a deceased relative

Need to transfer the title of my dad's car to my sister. I have a copy of the trust (I'm the executor) and death certificates from both parents. I know I need to file a small estate affidavit, and have questions about the form. I only need those questions answered & confirmation of what I will need to bring to the DMV.
