Fix ZenCart Checkout Error, "Some required elements of this page are missing"

One of our ZenCart customers, contacted us and said:
"I attempted about 6 times to place an order about 6 times via your web site earlier today however each time I did so all I received was a message "Loading". I attempted using two different credit card and each time received the same "loading" message. I allowed the system to run each time about ten minutes and kept getting the same results.
I know my internet connection is working as I placed several orders earlier with various vendors for assorted items."
I tested it from my side and I get the error shown in the attached screenshot.
Note: We use One Page Checkout module for ZenCart.
Can you see if you can figure out the issue with his ZenCart account?
Maybe it's an issue with his address in the zen_address_book table? or an issue with his record in the zen_customers table?
