Customized PDF Itinerary For Multi-Destination Travel

I'm in need of a skillful travel planner and copywriter to create a custom itinerary for a 2.5-week trip that includes 2-4 destinations. It must be in the format of a printed document that's easy to follow and carry or pull up on a PDF file. I will actually be doing the filling in of information but I need a clear and organized document to fill out with the necessary information. Can be pdf, WORD. I need to be able to fill it out. thank you
