OCR from Image to collect Medical Sensor Data

We have a medical app that allows us to capture data from Bluetooth devices. We are looking to add a new feature that allows the user to take a photo of of a medical device face plate. We want to have our app scan the image and find numeric values. We also want the app to find character/labels that indicate what the numeric value is associated with. For example, if we take a picture of a blood pressure devices, there will be three numbers (120, 80, 55 - these are examples). Near the 120 value in the image, you will see "sys". This means that 120 is the SYSTOLIC measurement of the blood pressure. For 80, you will see "dia". This means that 80 is the DIASTOLIC measurement of the blood pressure. And for 55, you might see "hr". This is the beats per minute (BPM or heart rate).

What I am looking for is a way to:
1- take a photo
2- scan the photo
3- find any numbers
4- finds any text - like "sys" or "dia"
5- associates the number to that text
6- assigns it to the correct field
7- allows the user to save the information

We have an existing Android and iOS app. You will be working with our developer to integrate this solution.

Please do not submit a bid if you are not an expert in OCR from photos.

The interface will need to look similar to the attached photo.
