Minimalist HTTPS Lead Capture Website -- 2

I'm in need of a minimalist design HTTPS (encrypted) lead capture website with deployment on hosting company. This website should capture the following user information:
MANY FIELDS PLEASE SEE ATTACHED DOCUMENT OVER 70 FIELDS AND SOME FIELDS ARE CONDITIONAL
The website should have a form where these fields can be filled out. This form submission should be sent to my email and also stored in a MongoDB database.
The site needs to have an OTP (one time passcode) verification system that only applies to users requesting a price quote. After the user provides their cell phone number, an OTP should be sent to that number for validation before the form can be submitted.
Ideal skills for this job include:
