Hi,

Please do not make an offer without reading the entire description, without seeing the code (link below) and without having the firm belief that you have all the technical capabilities to complete this project.

However what I need to add:

1 add/change some fields, to include in the email.

1 Possibility for the user to send a photo, with upload button or ideally with "drag here" and include the photo or a link to the photo in the body of the mail.

2 Possibility to save in a database, timestamp information, every time the form is used, to be used in a graphic. This graph should allow the user to adjust the time interval (as if it were a graph of a share price).

3. Some aesthetic level adjustments.

Finally, some support in the process of placing the script on a shared webhost.
