Problem for create multiples data on node.js and react project

we are having a problem here for add multiples data in nodes.js/react project... example
we need to create adress form like street, number, city eg... but on this form we need to add a button (+) plus for add more adress, duplicating the same fields from the previous address and saving and data base two adress.
we cant do this, some one help us?
