NodeJS Server Monitoring

I manage the nodejs app. I need to be able to monitor server metrics.
I want this implemented with the prom-client https://github.com/siimon/prom-client or collectd plugin
I need help with the instrumentation and need code well documented and instructions on how to integrate with the existing non-dockerized application.
