Node JS code not working as supposed to according to API docs

I am using QUINTADB service and trying to post data into database using RECORD > CREATE RECORD method described in the API documentation (https://quintadb.com/api/index#recordinfo). Post is partially working but it's not posting SUBFORM correctly. Need help fixing that NodeJS code.
