Insert new Record Into SQL Database, using Typescript, using Node.js, using Express, using Apollo Server for GraphQL

I need a fully functioning example of a web backend that inserts a new record into a SQL database.
* The code should use a model class of some kind.
* Some validation logic on the HTTP request should be included.
* Custom JSON should be included in the HTTP response.
* The code should be heavily commented for a reader who has never used the tech stack before.
* The tech stack should be:
