Create a web form based on an Excel sheet questionnaire and host it on Azure Web App
Budget: $30 – $250 USD
Summary: I have an Excel sheet with 8 Tabs. Each Tab has between 15 - 25 questions.
Each Sheet has about 10 columns.
Column A has the question #
Column B has a list of questions
Column C is blank
Column D contains multiple answers: Yes, No, I do not Know, Flag this question for later.
Column E is populated with text and it maps1:1 to Column D. For example: if you select Yes in Column D, then Column E may say “Good Practice”.
Column F has a fixed value of 0 or 1
Column G has a calculated value that must be kept the same in the Web form
Column H has fixed values “Required, Required and Addressable, Addressable
The plan is convert this Excel sheet into a Web Form that can be hosted on Azure. I need to be able to send people email invites to complete this forms after authenticating them to Azure AD.
I also need an Admin login where I would have admin capabilities where I can print, delete forms, export data, hide certain column.
More to come after we link with each other. We also will need to video conference to make sure we are on the same page. thank you
