Contact Details Submission Page

I need a clean, lightweight web page that lets visitors leave their full contact information—name, email, phone number, and physical address. Proper contact details are a must, so please add real-time or server-side checks that block obvious mistakes (e-mail format, sensible phone digits, no empty required fields).

When a user presses Submit, your code should instantly send a confirmation email to the address they provided. Storing each entry in a simple CSV file or a small MySQL table will be enough for now, as long as it’s easy for me to download later.

Keep it lean: a single responsive HTML/CSS/JS front end plus a small back-end (plain PHP, lightweight Node/Express, or a serverless function—whichever you prefer). No heavy frameworks. Comment the code so I can update wording or add fields in the future.

Deliverables
• Responsive contact page with all fields and validation in place
• Back-end script that logs submissions and triggers the confirmation email
• Short README covering deployment steps and any environment variables (SMTP, DB credentials)

I’ll sign off once I can submit test data, see the validation in action, and receive the confirmation email without issues.
