Build a prototype page for managing medical problems
Budget: $30 – $250 USD
Problem oriented medical records focuses on a patient's medical problems and omits all the extraneous, unnecessary information that's in traditional medical charts. As the name implies, it's focused around the patient's medical problems so that documentation can occur quickly and care be delivered logically. Thus, each patient record would consist of a list of medical problems and their corresponding codes from and ICD-10 database.
For starters, the software should be able to do the following:
--Each active problem is displayed in a list fashion and highlighted by a heading. These problems are selected via accessing an ICD10 disease database
--Headings could be color-coded, with active problems being green (green = go)
--Each problem would have brief and comprehensive views
--New problems would be added and would require more extensive description and data input would be quickened by the use of macros. Macros would also be used to input data on chronic problems as well (can be done later)
--Drop down arrow beside problem name would exist, with associated diagnoses and thus shared plans
--Within each problem, the SOAP concept, though not all components required for each visit and each problem
S

A
P
Within the "O" component (objective), show current and historical data such as physical exam findings, lab studies, and imaging studies
--Problems can be moved on the page, with arrows allowing up and down and also drag and drop
--Macro to quickly add one, few, or many problems at once and then "x" to delete problems non-applicable.
--Other needed data in the medical record (PSH, SH, FH, etc) to be displayed in margins
--Medication list maintained in the margins. I have a medication DB that can be modified/optimized
User to select which elements of the problem list would be printed.
So for each problem:
--Problem name and IDC 10 code would be displayed: "Hypertension (i10)"
--Problem can be designated active or inactive
--Active problems will appear at the top
--Inactive problems will appear at the bottom
--Problems can be displayed as showing all or showing only active
--Arrows will exist within problem heading allowing it to be moved up and down. Drag and drop will also be allowed to move problems
--A dropdown will exist beside the problem that will show associated diagnoses
--Problems will allow brief and detailed view with the ability to quickly change. At the top there will be a toggle to show brief and detailed view of ALL problems
Anatomy of EACH Problem:
S: Subjective
O: Objective
A: Comment/concern/DDx
P: Plan/Orders (disposition, diet, medications, labs, imaging, nursing, referral, instructions, misc, etc)
I am looking to develop this as a small prototype and if things go well, work with the same developer on the larger project. I would like to do the project in Node and perhaps Vue with a MYSQL DB. But again I'm not too concerned with that right now; I just want to get a simple, functioning prototype done of the problem-based approach.
If you have any questions just let me know
