Mikrotik ROS 7.x Dual WAN Configuration with Load balancing and failover

Create a complete configuration for Mikrotik ROS 7.x to accomplish these tasks:
Dual Wan load balancing using PCC
The connections coming from an internal IP and sent to an external ip have to be mantained on the same Wan connection to avoid sensible applications on remote site to force logoff the user due to change of ip.
Failover using two remote hosts for every Wan and send an email when a connection fails. Check every 3 minutes. When a connection fails, the traffic have to be routed to the other connection. The configuration have to check every five minutes if the connection is re-estabilished and activate again the load balancing and send an email to report the functioning connection again.
Additional informations:
The Wan connections use a private subnet ip address to communicate with the ISP's router. Isp routers have a dynamic public IPV4 address.
The config on mikrotik have to able to port publish a service inside the network. ISP's router forward all incoming traffic the the private ip assigned to Wan Mikrotik interfaces.

- Lan 192.168.11.1
ISP router2:
- Wan Dynamic
- Lan 192.168.12.1
Mikrotik WAN1: 192.168.11.2
Mikrotik WAN2: 192.168.12.2
Mikrotik Eth1: Wan1
Mikrotik Eth2: Wan2
Mikrotik Eth3-4-5 and wifi1-2 bridged: Lan 192.168.1.x (dhcp server on mikrotik)
