Netsuite CSV Import File Check for Child Matrix Items

I have an import file to import Child Matrix Items into Netsuite. I keep getting error messages and I need someone to review and fix the import sheet so that I can import it. The error message I get is either an invalid reference key or Matrix items parent must be a matrix item.
