mysql DB with health care information

Hello. I have a database in mysql and needs to create forms to fill database. Main table will contain information digited (as name, date of birth ...) and other information will be filled with information from other tables. After filling the info, the info will need to be shown in a webpage in certain order which I will tell you. I hope you all understand. I am no good explaining very well. But the idea is to have forms for filling certain tables and one main form to fill with client information which should contain data from other tables in same database plus personal information of the client.
