Website with Map, Form for User to Submit, Visualisation of User-Submitted Info

Website:
a) Real map
Two functions for this website:
i) User can submit data
ii) User can view the data already submitted by other users (visualisation)

b) Submit Data (Form)
- Wherever the user is, outdoors or indoors, when they see a pest, they can come to our website (www.pestsmany.com for example), and there is an option for them to submit a form.
- Details they need to enter:
(1) location of accident/incident, either by auto calc from phone GPS or enter postal code/address,
(2) drop down list of pests,
(3) select box or select button on state of pest [alive or dead],
(4) date (either datetimenow or user can manually enter a date and time),
(5) submit photo or video [optional]
(6) User's name and contact details (email OR phone)
- All submitted info needs to be approved/moderated by us. Therefore an interface to carry out review and approval/reject function (single/bulk)

c) View Data / Browse Map
- The website will display all 'allowed' data on the website/map, based on accumulated data/reports from public.
- Options for user when browsing should include, time period (can select start and end dates), type of pests (checked boxes, can view 1 pests, selected pests, or all pests).

Pests:
- Cockroaches
- Flies
- Rodents
- Mosquitoes

These for now, but over time, we should be able to add new pests (on both Form submission and also visualisation).
