SASS based Risk Assessment for Vendors -- 2

We require a SASS-based risk assessment tool to evaluate our vendors. The primary purpose of the tool is vendor evaluation, and we have a defined set of criteria that we would like to use for this evaluation. We also require a moderate level of customization for this tool.
