MySQL Database Development and User Interface for Gym App

I have an idea to create a gym app that groups users based on certain preferences so they can find people to train with.

I have asked 50 people to answer the questionnaire below.

I need a MySQL database developed to store this data and queries created to group clients based on the following criteria:
(1) the gym they attend.
(2) their preference of gender for training partners.
(3) the time of day they train.

I also require a user interface to be created.
