Form and database

Hi I need to build a form that will be completed from a phone's browser that enters data into a MySQL database. So it needs to be mobile friendly Bootstrap perhaps?

These are the fields and input types for the form / database.

Driver Name (Input Field)

Truck (Dropdown)
Grace
Peace
Hope

Pickup Location (Dropdown)
Amazon
Wegmans
Lidl
Target
Costco

Category (Dropdown)
Produce
Meats
Misc. Frozen
Dairy
Misc. Refrigerated
Misc. Dry Food
Bread/Bakery
Non-Food Dry
Beverages

Pallets Received (Input Field)

Or

Pounds (Input Field)

Temperature (Input Field)

Date / Timestamp – Get Automatically from the system

Disclaimer by submit button – Please make sure doors and lift gate are locked.


The form may be used multiple times to enter different categories from the same pick up location. It would be nice if the Driver Name, Truck and Pick up location could be saved per session. Maybe a button by submit that says add more.


Then there should be an admin portal that will be mostly accessed via a desktop browser. This is just another (similar) page with access to the database.
This should have the following:

Ability to enter data – Same as the form above.
Has additional fields:
Donor Name (Input Field) After Pick Up location

Button to export to Excel
Button to Empty Database
Ability to add/remove trucks, locations and categories to dropdown menus


Please let me know if anyone can do this. This is for a non profit organization that gets food donation from companies and they in turn distribute it for free to people in local communities that have no food.
