Easy app for Add record on remote mysql db (2 fields)

I need an easy app for add some record on mysqldb
I wish just a field where I can write test and app must create an insert into query to db

If field 1 is XXXXXXXXXX and field 2 is YYY query will be something like this:

insert into tablename (name1,name2,name3,name4) values ("YYYY","0000","0BXXXXXXXXXX00002359FF00005012310000900000","0");

That's all

* Parameter of mysql connection must be incrypted on local parameter txt file

I have not preferences if C++, C#, APS, Java it will be run on windows 11
