Connect a MySql Database

I am trying to set up a Report Writer for MySQL Database.
My MySql Database is hosted on Godaddy and I can show you the Database. I also have the Server Name, User Name and Password that I use to access the tables with PHP.
The Report Writer is Seal Report Designer. The setup screen is looking for an address for my Database.
I need someone who can figure out the address of my GoDaddy base so the Report Writer can connect to it.
